Check – made out to UUCB and mailed to Unitarian Universalist Church of Buffalo, 695 Elmwood Ave, Buffalo, NY 14222 with honoree/deceased person’s name and designation in memo line. Please fill out and include this form.
For gifts made in honor or memory of UUCB members, the honored designee or a family member of a deceased member being remembered by way of a gift will be notified of the donor’s beneficence.
